Please use our Order Form to organize your orders. We need to have your items grouped and totaled according to style, color
and size. Clubs, Teams, Schools, Churches and any group order needs to come in on one form and be paid for with one
check.
Our Order Form is a WORD Document so you can type right into it. To order, please save it with
YOUR NAME to identify it (instead of "flyinlionsorder") and email it back to us as an attachment.
If you are
bringing in a 4-H jacket for embroidery, please fill out the Jacket Planner Form ahead of time and have your thead colors in mind.
